Medical Waiver

By ordering this item, I am certifying that:

I am at least 18 years old.

I have been advised by Injoyhearing.com that the Food and Drug Administration has determined that my best health interest would be served if I had a medical evaluation by a licensed physician (preferably a physician who specializes in diseases of the ear) before purchasing a hearing aid.

Lastly, I acknowledge that none of the below conditions or symptoms apply to me and that the existence of such conditions would prohibit Injoy from selling a hearing aid to me without medical clearance:
• visible congenital or traumatic deformity of the ear:
• history of, or active drainage from the ear within the previous ninety (90) days.
• history of sudden or rapidly progressive hearing loss with in the previous ninety (90) days.
• acute or chronic dizziness
• unilateral haring loss of sudden or recent onset within the previous ninety (90) days.
• air bone gaps.
• visible evidence of bleeding, significant earwax accumulation, or a foreign body in the ear canal; or
• pain or discomfort in the ear.

I do not wish to obtain a medical evaluation before purchasing a hearing aid.

Bill of Sale

By completing your purchase, you agree to be bound by the terms in this Bill of Sale and Injoy Hearings Terms and Conditions available at www.injoyhearing.com/terms-and-conditions.

Warranty Period and Covered Services:
Injoyhearing.com provides three (3) years of repair warranty and three (3) years of loss and damage coverage for all Injoy hearing aid purchases.
All product warranties are in effect from the date of the purchase. Should repair or replacement of a Injoy hearing aid product be necessary, we will make every effort to provide this service in a timely manner. Please return the product with a completed Repair/Remake Service form (if required) to Injoy in appropriate packaging. A product repaired under warranty maintains the original purchase warranty effective date.

Loss and damage policy:
Any hearing aid that is stolen, lost or damaged can be replaced while the product is under the
loss and damage policy. The following fees apply:
 Injoy hearing aid per unit deductible: $295
 Injoy charging case: $175
A hearing aid may be replaced one time under this program and may not be returned for credit. Should a claim need to be filed, contact Injoy support. Injoy may require that you return the defective or damaged device before sending you a replacement.

Return Policy:
You have the right to cancel this purchase for any reason at any time before the forty-fifth day after delivery to your specified address of the hearing aid(s). Return the hearing aid(s) in the same condition. Injoy will pay for return shipment. See our Terms and Conditions for further details. You must contact Injoy at 1-844-554-6569 to obtain a return authorization.

I understand a hold will be placed on my card for the full amount of the purchase and should I not return the product within 30 days I will be charged the full amount. I understand my card will be charged $1 to confirm authorization.
